Job summary

Brookdale seeks compassionate caregivers to provide dementia care and meaningful engagement for residents in a supportive community. You will follow person-centered plans, assist with daily living activities, and collaborate with a multidisciplinary team to promote dignity and independence.

Responsibilities include documenting care, supporting meals and hydration, leading engagement activities, and maintaining a safe, clean environment while adhering to state regulations and Brookdale policies.

Qualifications

• High school diploma or GED and related experience preferred in care with older adults living with dementia.
• Knowledge of dementia, Alzheimer's type, and person-centered care.
• Ability to read and follow care plans, document accurately, and communicate effectively.

Responsibilities

• Provides dementia care including activities of daily living and meaningful engagement per resident's service plan.
• Assists residents with daily living activities and supports independence.
• Participates in team discussions and understands residents' Life Story and preferences.
• Serves meals, supports proper nutrition, and encourages hydration while respecting resident choice.
• Initiates Daily Path engagement programs and encourages social participation among residents.
• Maintains accurate daily records including care notes and observations for supervisor review.
• Follows medication procedures where permitted by state regulations and reviews physician orders.
• Keeps environment clean, safe, and orderly and adheres to cleaning schedules.
• Responds to emergencies and resident requests promptly, including pull cords and family communications.
• Monitors resident safety and reports changes to supervisor; collaborates with coworkers.
