Qualifications

• Education: High School Diploma or GED; program graduate in Nursing or Allied Health preferred, Associates Degree in Nursing or Allied Health preferred.
• Experience: 2 years Healthcare experience in related field (Required).
• Licenses and Certifications: CPR/BLS Certification within 30 days; LPN/PTA/COTA licenses (as applicable) (Required).

Responsibilities

• Performs intake and manages the end-to-end referral process for Martha W. Goodson Center services and programs by collecting and reviewing required information, determining referral appropriateness based on established criteria, and coordinating service delivery through appropriate team assignment.
• Facilitates communication and information sharing across stakeholders, arranges transitions to external agencies when referrals are not accepted, and ensures follow-up with all parties until services are initiated, completed, or closed.
• Utilizes clinical knowledge to gather and document accurate, program-specific referral information while managing inquiries in line with service standards.
• Acts as a resource on available services, communicates eligibility criteria and options to patients, caregivers, and referral sources, and supports coordinated, patient-centered care.
• Maintains data integrity by entering, updating, and retrieving information across multiple systems with timely documentation in medical records.
• Ensures all required records and forms are completed per standards to support efficient operations and continuity of care.
• Maintains knowledge of Martha W. Goodson Center services, healthcare regulations, and community resources to support referrals.
• Stays informed of updates affecting service delivery and collaborates with teams to strengthen community relationships.

Education
