Qualifications

• 2 years healthcare experience in related field (Required)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute within 30 Days (Required)
• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) (Required) or License Physical Therapy Assistant (PTA) - VDHP (Required) or Licensed Occupational Therapy Assistant (COTA) - NBCOT (Required)

Responsibilities

• Performs intake and manages the end-to-end referral process for Martha W. Goodson Center services and programs by collecting and reviewing required information, determining referral appropriateness based on established criteria, and coordinating service delivery through appropriate team assignment.
• Utilizes clinical knowledge to gather and document accurate, program-specific referral information while effectively managing incoming inquiries in alignment with service excellence standards and communicates this information clearly to patients, caregivers, and referral sources.
• Maintains data integrity and accuracy by entering, updating, and retrieving information across multiple systems and ensuring all required records and forms are completed in accordance with organizational standards.
• Maintains a comprehensive and current knowledge base of Martha W. Goodson Center services and programs, relevant healthcare regulations, and community resources to support effective referral coordination and patient access to care.
