Job summary

Riverside Health System in Newport News, VA seeks a Registered Nurse to assess PORT participants and coordinate care under supervision of the practice manager and Director. The RN will develop care plans, coordinate referrals, and ensure high-quality patient education and discharge planning.

The role requires an RN license in VA (or eligibility), an Associate Degree at minimum, and CPR/BLS certification upon hire. Weekend/irregular shifts are required and care across multiple sites may occur.

Qualifications

• Associates Degree in Nursing required; Bachelor’s preferred.
• 2 years RN experience required.
• CPR/BLS certification required upon hire.

Responsibilities

• Assess patient clinical and psychosocial needs per standards of practice for PORT.
• Collaborate with patients and families to develop and evaluate care plans.
• Coordinate care with interdisciplinary team to ensure safe transitions.
