Job summary

Hospice & Community Care is seeking a Dementia Coach to join our team. This full-time role focuses on educating patients, families, and caregivers across dementia stages, from early involvement to end-of-life care, and coordinating with admission teams to place appropriate patients in dementia care.

You will lead community education efforts, deliver Teepa Snow Positive Approach to Care training, and supervise the Virtual Dementia Tour program, expanding access to our services across the

Qualifications

• Associate’s degree or equivalent in related field required.
• Teepa Snow Dementia Coach certification required within 1 year from date of hire.
• Virtual Dementia Tour Certified trainer required within 1 year from date of hire.
• 1 year of experience working with patients and caregivers living with dementia required.
• Must be vaccinated for influenza or have a religious/medical exemption.

Responsibilities

• Identify appropriate patients for dementia care with admission teams.
• Provide education and coaching to patients, families, and caregivers across disease stages.
• Coach caregivers on practical strategies for dementia-related symptoms.
• Support families with anticipatory guidance related to progression and end-of-life changes.
• Consult with IDT to troubleshoot and guide care.
• Lead the Dementia Resource Team as a key organizational resource.
