Job summary

Haven Hospice is seeking a Dementia Program Activities Coordinator to engage dementia patients in their homes and facilities, modeling activities and bathing strategies for caregivers. You will work as an IDT member, coordinating care plans and documenting progress in the EMR to support a compassionate, interdisciplinary team.

The role requires a CNA background with 1–2 years of experience and a high school diploma or GED.

Qualifications

• High School diploma or GED equivalent.
• 1–2 years CNA experience preferred.
• Experience working with interdisciplinary team preferred.

Responsibilities

• Engage dementia patients with activities in home and facility settings.
• Model bathing strategies to maximize comfort for patient and caregiver.
• Coordinate with IDT and share information with care team.
• Create customized activity guides and ADL plans.
• Document visits in EMR and share with care team.
• Bi-monthly follow-ups to assess engagement strategies.
