Job summary

Virginia Commonwealth University Health is seeking a Dementia Practitioner to join the GUIDE program. You will complete comprehensive assessments, develop care plans for patients with dementia, and coordinate with caregivers and the patient’s care team across the continuum in the community and clinics.

You will provide dementia training, data collection, outreach, and presentations, plus screen, triage, referral care, and monitor participants.

Qualifications

• Bachelor’s Degree in a Human Services field or 2–3 years of Community Health Worker care coordination experience.
• Master’s Degree in gerontology, nursing, or social services preferred.
• Certified Dementia Practitioner certification and at least 10 hours of dementia care coordination training/certifications.
• Minimum two years of gerontological/intervention delivery work or administrative/coordinator experience.

Responsibilities

• Complete comprehensive assessments and implement care planning for beneficiaries with dementia and caregivers.
• Provide dementia patient and caregiver case management and coordination with the care team.
• Deliver GUIDE program trainings, presentations, and community outreach.
• Screen, triage, refer, and monitor dementia care needs; recruit participants for the GUIDE program.
• Maintain documentation, data entry, and record keeping; support events and special projects.
