Job summary

AHRC Nassau is seeking a dedicated caregiver for an Intermediate Care Facility role. You will assist residents with daily personal care, support independence, and participate in an interdisciplinary team to plan and monitor progress.

The position involves aiding with meals, mobility, safety, and recreational activities, with potential AMAP duties for medication administration when qualified. A NYS driver’s license is required.

Qualifications

• High School Diploma or Equivalent preferred.
• Six months experience with DDI population preferred.
• NYS driver’s license required and maintained throughout employment.

Responsibilities

• Assists residents with personal hygienic needs (washing, toileting, feeding, showering) and may transport to medical appointments.
• Implements resident-specific programs to promote independence and goal achievement.
• Assists with prescribed diets and portion control to maintain ideal weight.
• Administers medications and medical procedures if AMAP certified and per doctors orders.
• Maintains resident safety through supervision and adaptive equipment as needed.
• Provides ongoing supervision and supports residents’ safety and well-being.
• Implements recreational activities (arts/crafts, games, sports) to meet recreational needs.
• Attends and participates in inter-disciplinary team meetings to discuss progress and share information.
• Purchases clothing, grooming items, and personal necessities to meet daily living needs.
