Job summary

Northwestern Community Services Board is seeking a Developmental Disabilities (DD) Case Manager to support individuals with developmental disabilities across our jurisdiction. The role involves assessing needs, developing service plans, arranging residential placements, and coordinating with service providers.

The position requires an understanding of DD services, a valid driver’s license, and the ability to travel within the catchment area.

Qualifications

• Bachelor’s degree earned from an accredited college or university in a relevant human services field is required.
• At least one year of experience working in DD services to meet DMAS requirements is required.
• Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.

Responsibilities

• Assess individuals’ comprehensive needs for services and supports through interviewing individual and/or family.
• Develop individualized written case management services plans which outline service needs.
• Link individuals to services and supports specified by the service plans.
• Maintain complete and accurate individual records in accordance with regulations.
• Coordinate, monitor, and advocate for individuals’ receipt of required services from providers.
• Provide supportive intervention and follow-up with individuals and families.

Education

Bachelor’s degree in a relevant human services field
