Job summary

Northwestern Community Services Board is seeking a Developmental Disabilities Services Intake Specialist to coordinate the assessment and intake process for individuals with developmental disabilities and their families in Virginia.

You will build partnerships with community resources, conduct eligibility assessments, and maintain records in the Virginia Waiver Management System to support DD waiver eligibility and access to services.

Qualifications

• Bachelor’s degree in a relevant human services field is required.
• At least five years of experience working with DD individuals.
• Must possess a valid driver’s license and be able to provide your own transportation.

Responsibilities

• Develop and maintain good working relationships with community partners and agencies to coordinate services.
• Assess individuals for DD services and determine eligibility and priority levels.
• Enter and maintain data in the Virginia Waiver Management System (VWMS).
• Provide education, resources, and crisis intervention as needed and serve on project committees.
