Job summary

Jackson County Health & Human Services seeks a Case Manager to design individualized service plans for people with developmental and intellectual disabilities, coordinating Medicaid services and monitoring supports.

Position requires a bachelor’s degree in behavioral or social sciences and related experience, plus a valid driver’s license. Spanish-speaking applicants are encouraged to apply; union SEIU represents this role.

Qualifications

• Bachelor’s degree in behavioral science, social science, or closely related field and two years experience in developmental disabilities.
• Any combination of education and experience providing the applicant with the desired skills.
• Requires possession of a valid driver’s license and an acceptable driving record.
• Bilingual Spanish-speaking applicants are encouraged to apply.

Responsibilities

• Provides a variety of case management services to a developmentally disabled population, coordinating, monitoring and providing services, and authorizing Medicaid services.
• Uses needs assessment tools to assess client needs and determine level of care and services.
• Determines eligibility for services and develops individualized case plans.
• Participates on Individual Support Teams and coordinates Medicaid services.
• Conducts home visits, escorts clients to appointments, and maintains progress notes and reports.
