Job summary

Qualifications

• Requires BA degree in Social Welfare or related field, or equivalent.
• One year of experience in a like position may be considered in lieu of degree.

Responsibilities

• Provide case management services to an assigned group of persons with developmental disabilities.
• Develop and monitor Individual Program Plans.
• Coordinate services with other agencies and service providers.
• Conduct home visits with clients, families or other care providers.
• Consult with clinical and other professional staff.
• Advocate on behalf of clients.
• Complete reports and forms.
• Maintain case records.
