Job description

Developmental Disabilities (DD) Case Manager

The Developmental Disability (DD) Case Manager serves to assist in the provision of services to developmentally disabled individuals of the jurisdiction(s) served by his/her clinic site position location. Primary duties include individual assessment, service plan development, residential placement and follow-up, information and referral, case recording, and liaison with other agencies and facilities which serve developmentally disabled individuals.

Qualifications
  • A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
  • Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
  • At least one year of experience working in DD services to meet Department of Medical Assistance (DMAS) requirements required.
  • Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.
Salary

Starting at $52,788
