Job summary

University Hospitals Pain Management seeks a dedicated Social Worker II to join the Seidman Cancer Center in outpatient oncology care in Mentor/Concord. This full-time, dayshift role supports cancer patients and families within a dynamic, interdisciplinary team.

Collaborate across care teams to assess psychosocial needs, coordinate discharge planning, and connect patients with community resources while maintaining PHI and UH policies.

Qualifications

  • Provides psychosocial assessment to assist patients and families (including clinically complex).
  • Develops, documents and implements plans based on psychosocial needs and discharge needs.
  • Develops plans with attention to individual patient and family goals.
  • Links to community resources and UH network services; liaises with community agencies.

Responsibilities

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals.
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
    Develops plans with attention to individual patient and family goals
    Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.
