University Hospitals in Cleveland is seeking an outpatient Social Worker to join the Seidman Cancer Center. This full-time, dayshift position supports oncology patients and collaborates with an interdisciplinary team to coordinate psychosocial care and discharge planning.

The role emphasizes psychosocial assessment, age-appropriate care, linkage to community resources, and serving as a liaison to community agencies, with opportunities for professional development and generous benefits.

Qualifications

  • Requires ability to assess psychosocial needs and provide appropriate interventions.
  • Experience working with oncology patients is preferred.
  • Ability to develop and document comprehensive discharge plans.

Responsibilities

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals.
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
