Armor Health seeks a Licensed Practical Nurse (LPN) to join our team at the Corrections Center of NW Ohio in Stryker, OH. You will deliver dedicated nursing care in a correctional setting and support a dynamic healthcare team.

You will assess patients, assist with exams, administer TB tests, collect vitals, and coordinate transfers while documenting in the POMR system. Collaboration with providers is key to safe, timely care.

Qualifications

• Current/Active state license as an LPN.
• Current CPR certificate.
• Proficient with electronic medical records.
• Ability to pass pre-employment Level 2 clearance, background check and drug screen.

Responsibilities

• Provide holistic nursing care to patients in a correctional setting.
• Assist healthcare providers with examinations, treatments, and routine procedures.
• Obtain patient histories, vital signs, and administers TB skin tests.
• Conduct lab tests and ensures the results reach the physician or nurse.
• Ensure patients receive appropriate care for injuries or conditions.
• Review patient charts and report observed symptoms, reactions, and changes in patient\'s conditions.
• Coordinate patient transfers to medical facilities or outside health care providers; administer medications; document contacts using the POMR format.

Responsibilities

• Provide holistic nursing care to patients in a correctional setting.
• Assist health care providers, registered nurses, and allied health professionals with examinations, treatments, special tests, and routine procedures.
• Obtain patient health history, laboratory specimens, vital signs and administers TB skin tests.
• Conduct lab tests and ensures the results are delivered to the correct physician or registered nurse.
• Ensure patient is receiving proper care for injury or condition.
• Review patient chart; records and reports observed symptoms, reactions, treatments, and changes in patient's conditions.
• Coordinate patient transfers to medical facilities or outside health care providers; administer medications; implement healthcare practitioners' orders; and document all patient contacts using the POMR format.
