Armor Health is seeking a dedicated Licensed Practical Nurse (LPN) for the Corrections Center of NW Ohio (CCNO) in Stryker, OH. Join a team delivering holistic care in a secure, correctional environment.

You’ll support providers, perform nursing tasks, and document care with precision, ensuring safety and adherence to clinical protocols.

Qualifications

• Current/Active state license as an LPN.
• Current CPR certificate.
• Proficient with electronic medical records.
• Ability to pass pre-employment Level 2 clearance including background check and drug screen.

Responsibilities

• Provide holistic nursing care to patients in a correctional setting.
• Assist health care providers, RNs, and allied professionals with examinations, treatments, tests, and procedures.
• Obtain patient health history, collect specimens, take vital signs, and administer TB skin tests.
• Conduct lab tests and ensure results reach the designated physician or RN.
• Ensure patients receive appropriate care for injuries or conditions.
• Review charts, records, and report observed symptoms, reactions, and changes.
• Coordinate transfers to medical facilities; administer medications; document contacts using POMR.

Responsibilities

• Provide holistic nursing care to patients in a correctional setting.


• Assist health care providers, registered nurses, and allied health professionals with examinations, treatments, special tests, and routine procedures.


• Obtain patient health history, laboratory specimens, vital signs and administers TB skin tests.


• Conduct lab tests and ensures the results are delivered to the correct physician or registered nurse.


• Ensure patient is receiving proper care for injury or condition.


• Review patient chart; records and reports observed symptoms, reactions, treatments, and changes in patient's conditions.


• Coordinate patient transfers to medical facilities or outside health care providers; administer medications; implement healthcare practitioners' orders; and document all patient contacts using the POMR format.
