Job summary

Waystone Health & Human Services is seeking a Day Habilitation Program Supervisor in Haverhill, MA to manage daily operations and ensure delivery of safe, compliant services for individuals with ID/DD. You will supervise staff, coordinate program activities, and maintain regular communication with families and guardians.

The role requires a Bachelor's in Human Services or related field (or equivalent experience) and a minimum of two years of supervisory work within ID/DD.

Qualifications

• Bachelor’s degree in Human Services or related field with at least three (3) years of experience within the field of ID/DD.
• High School Diploma or GED with at least five (5) years of experience within the field of ID/DD.
• Bilingual proficiency in English and Spanish preferred.
• Minimum of two (2) years of supervisory experience.
• Minimum of two (2) years of experience implementing, developing and monitoring ISPs or similar plans.
• Experience with Electronic Health Records (EHR) platforms, such as eHana, preferred.

Responsibilities

• Hire, supervise, evaluate and train Case Managers and Developmental Specialists.
• Create and implement curriculums and activities that support the interests and needs of individuals served and ISP goals.
• Coordinate monthly Interdisciplinary Team Meetings for clinical, medical and behavioral services.
• Ensure timely submission of program data for individuals receiving services.
• Develop, implement and review Day Habilitation Service Plans (DHSPs) and ISPs.
• Monitor implementation of approved strategies to ensure compliance and positive outcomes.
• Coordinate referral and intake processes, tours and funding communications.
• Maintain knowledge of CARF, OQE, DDS, MH, ABI policies and compliance.
• Partner with Director and Community Engagement Coordinator on member-driven services.
• Assist with annual department budget development and monitoring.
• Oversee facility safety, cleanliness, inventory and liaison with families/guardians.
• Participate in inspections, audits, and trainings.
