Job summary

S:US in New York is seeking an Assistant Program Director for Day Habilitation to lead person-centered planning and day program operations. This role blends QIDP expertise with management duties to promote community integration, meaningful activities, and regulatory compliance.

The ideal candidate will have a Bachelor's in Human Services, at least 1 year as a QIDP with IDD, and bilingual proficiency. The position offers a stable salary and comprehensive benefits.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Minimum 1 year experience working with individuals with Intellectual and Developmental Disabilities (IDD).
• Minimum 1 year experience in the role of QIDP with individuals with IDD.
• Demonstrated knowledge of day habilitation services, OPWDD regulations, and Medicaid billing processes.
• Experience in staff supervision, person-centered planning, and program management preferred.
• Bi-lingual is a must

Responsibilities

• Lead development and implementation of individualized person-centered plans that align with Life Plans.
• Oversee daily operations of the Day Habilitation program ensuring quality services and regulatory compliance.
• Support and mentor Direct Support Professionals through training and coaching.
• Assist Program Director with program management, staff supervision, and on-call responsiveness.
• Promote continuous quality improvement and advocate for rights, dignity, and independence.
• Coordinate Staff Action Plans reflecting meaningful, person-centered outcomes.
• Advocate for persons supported during Life Plan meetings and ensure their voices are represented.
• Participate in screening, admission, and discharge processes to ensure smooth transitions.
