Job summary

Elara Caring invites you to join our Home Health Physical Therapy team serving the Danbury, Connecticut territory. This field-based role emphasizes one-on-one patient care, independence in the field, and teamwork with a clinical staff.

A base salary ranges from $100,000 to $115,000 with a sign-on bonus available. You will evaluate mobility, design individualized plans, educate patients and caregivers, and document progress while traveling to patients’ homes.

Qualifications

• Graduate of an American Physical Therapy Association-approved physical therapy program.
• Active and unrestricted Connecticut Physical Therapist license.
• At least one year of experience in a healthcare setting.
• Strong communication, organization, and patient-care skills.

Responsibilities

• Evaluate patients’ mobility, strength, safety, and functional needs.
• Develop and implement individualized treatment plans.
• Help patients improve mobility and safely perform daily activities.
• Educate patients and caregivers on exercises, equipment, and safety techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate changes in condition to physicians and members of the care team.
• Complete accurate and timely clinical documentation.
• Collaborate with nurses, physicians, therapy professionals, and other team members.
• Support patients in reaching their highest possible level of independence.
