Elara Caring is seeking a Physical Therapist to provide one-on-one home health care in the Danbury, CT area. This field-based role lets you manage patient visits with support from a clinical team, delivering personalized care in patients' homes.

You’ll evaluate mobility, develop tailored treatment plans, educate patients and caregivers, and document progress while collaborating with the care team. The role offers a competitive base salary and robust benefits, including insurance and 401(k).

Qualifications

• Graduate of an American Physical Therapy Association-approved physical therapy program.
• Active and unrestricted Connecticut Physical Therapist license.
• At least one year of experience in a healthcare setting.
• Strong communication, organization, and patient-care skills.
• Ability to work independently in a home-based environment.
• Willingness to travel throughout the assigned Danbury territory.
• Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.

Responsibilities
