Job summary

Elara Caring is hiring a Physical Therapist to provide in-home care along the Connecticut Shoreline and into parts of New Haven. You will evaluate mobility, develop individualized treatment plans, and deliver one-on-one patient care in patients' homes across East Haven, Branford, Guilford, Madison, and surrounding communities.

You’ll have autonomy to organize your patient visits, support from a collaborative clinical team, and a base salary of $105,000–$115,000 with a $5,000 sign-on bonus.

Qualifications

• Graduate of an American Physical Therapy Association-approved physical therapy program
• Current and unrestricted Connecticut Physical Therapist license
• Strong communication, organization, and patient-care skills
• Ability to work independently in a home-based environment
• Willingness to travel throughout the assigned territory
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes
• Ability to frequently lift, push, pull, and support up to 50 pounds

Responsibilities

• Evaluate patients’ mobility, strength, balance, safety, and functional needs
• Develop individualized treatment plans with practical, patient-centered goals
• Provide skilled physical therapy in patients’ homes
• Help patients move safely and perform daily activities with independence
• Monitor progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises and safe mobility
• Collaborate with physicians, nurses, caregivers, and other therapy professionals
• Complete accurate and timely clinical documentation
