Elara Caring is seeking a Physical Therapist for a Home Health role in Lancaster, Massachusetts. You’ll provide direct patient care, evaluate functional needs, and design individualized care plans, working with a collaborative, supportive team to achieve optimal outcomes.

Formación

• Graduate of an APTA-approved Physical Therapy program.
• Current, unrestricted PT license in Massachusetts.
• Minimum 1 year of healthcare experience.
• Ability to travel within Worcester and surrounding areas (50% travel).

Responsabilidades

• Provide direct patient care, evaluate functional needs, and design individualized care plans.
• Collaborate with other healthcare professionals to ensure optimal outcomes.
• Document evaluations, treatment goals, and progress.
• Act as a resource and promote physical therapy in the community.
