Job summary

Elara Caring is seeking a Physical Therapist to provide home health services in the Danbury, Connecticut territory. This field-based role offers autonomy to manage patient visits with support from a clinical team.

You will evaluate mobility, develop individualized plans, educate patients and caregivers, and document progress. Travel up to 50% of the time and build meaningful relationships in patients' homes.

Qualifications

• Must have DPT or eligibility for PT licensure in CT.
• Strong communication and organizational abilities.
• Ability to work independently in home-health settings.

Responsibilities

• Evaluate patients’ mobility, strength, safety, and functional needs.
• Develop and implement individualized treatment plans.
• Educate patients and caregivers on exercises, equipment, and safety techniques.
• Monitor progress and adjust plans as needed.
• Document clinical progress accurately and timely.
• Collaborate with nurses, physicians, and therapy team.
• Travel to patient homes and represent the care team in the Danbury territory.
