Job summary

The WISe (Wraparound with Intensive Services) Care Coordinator at YMCA of Greater Seattle leads youth and family teams through the wraparound process, coordinating care plans and services for clients with complex needs across systems such as criminal justice, mental health, and foster care.

Qualifications

• Bachelor's degree in psychology, social work or related social services field or equivalent experience; Masters preferred.
• Experience with teens or young adults in employment, foster care, housing, education, or mental health settings.
• Ability to teach independence skills and plan supportive activities.
• Strong youth engagement and relationship-building skills.

Responsibilities

• Facilitates WISe team meetings and guides the WISe process.
• Provides intensive case management for a caseload of 10-12 WISe clients.
• Delivers services using a strengths-based, family-focused, culturally competent approach.
• Creates and reviews individualized care plans and tracks progress.
• Coordinates with WISe Therapists and WISe Peer Partners.
• Ensures confidentiality and timely documentation.
