Job summary

Catholic Charities of Baltimore Weinberg Housing and Resource Center is seeking a Crisis Prevention Coordinator to support clients facing crisis, housing instability, and behavioral health needs. You will provide crisis intervention, stabilization, and connections to resources while collaborating with case management and community partners.

The role requires a Bachelor’s degree in Human Services or related field and at least two years of experience with at-risk populations.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or a related field is required.
• Minimum of two years of related professional experience with at-risk populations.
• Experience working with individuals in crisis and familiarity with behavioral health, trauma, or housing instability.

Responsibilities

• Build therapeutic relationships with clients and collaborate with Case Management for care planning.
• Promote housing stability by implementing interventions to reduce recidivism and increase retention.
• Provide crisis intervention and coordinate referrals to medical or mental health resources.
• Evaluate hospital drop-offs for appropriate care and continuity of services.
• Engage clients to reduce emergency service utilization and connect to supports.
• Lead educational groups and provide individual guidance on mental health and substance use.
• Develop partnerships with external providers to strengthen coordinated care.
• Maintain timely documentation of all client interactions and outcomes.
• Adhere to agency policies and participate in ongoing professional development.
• Perform other duties as assigned.
