Qualifications

• Highschool diploma or GED required.
• Associates degree in a behavioral health related field preferred.
• Case management experience preferred.
• Minimum six months of experience in multiple settings and levels of care; one year preferred.

Responsibilities

• Coordinate care and discharge planning for high-utilizer crisis patients.
• Assist patients in obtaining clinically appropriate services and reduce barriers to treatment.
• Coordinate with all involved parties; report to Clinical Director.
