Lutheran Family Services Rocky Mountains is seeking a Family Stabilization Specialist to provide crisis mental-health case management for refugees and other high-need populations in Colorado. You will coordinate care plans, connect clients to community resources, and collaborate with staff to promote stability and well-being amid complex family dynamics.

The role requires strong advocacy, cross-cultural communication, and thorough documentation in a fast-paced team environment.

Competenze

• Experience providing case management to individuals in crisis (preferably in Colorado).
• Ability to adapt to a fast-paced environment with flexibility and resilience.
• Knowledge of refugee resettlement practices and local resources for crisis clients.
• Cross-cultural sensitivity and ability to work with diverse populations.
• Strong organizational skills and thorough documentation.

Mansioni

• Advocate for refugee clients and ensure equal treatment for all client groups.
• Develop Plans of Care with input from refugee staff and clients.
• Screen referred clients to arrange appropriate services.
• Provide crisis navigation, psychoeducation, and update Plans of Care.
• Coordinate translation and interpretation services as needed.
• Maintain professional client case notes and documentation.
• Collaborate with FSS team on trainings and reporting.
• Ensure compliance with HIPAA and agency contracts.
