Job summary

Community Housing Innovations (CHI) is seeking a Service and Care Coordinator to establish clinical partnerships and connect clients to mental health and substance use supports. You will lead group discussions, coordinate services, and support crisis response for children and adults within our housing programs.

The role requires an MSW, NYS LCSW with SIFI, and 2-5 years of relevant experience. Strong communication, organization, and the ability to work independently are essential, along with

Qualifications

• Master of Social Work required from an accredited university.
• New York State Licensed Clinical Social Worker, SIFI.
• 2-5 years providing clinical and educational services in an approved setting.

Responsibilities

• Develop clinical partnerships with external providers and organizations for mental health, substance use, and trauma recovery.
• Provide linkages to community-based mental health care and psycho-social supports with referring agencies.
• Lead monthly or quarterly group discussions and case conferences across the organization.
