Job summary

Paycom is seeking a Clinical Client Care Coordinator (LMSW/LMHC) under the Sr. Director of Housing to deliver intensive case management and crisis intervention.

Qualifications

• Master’s degree in Social Work or related clinical field required.
• LMSW or LMHC licensure required within 90 days of employment.
• Experience delivering clinical services, counseling, case coordination or supportive services.
• Experience conducting psychosocial assessments and developing service plans.
• Experience facilitating groups and developing group curricula.

Responsibilities

• Conduct comprehensive psychosocial and resident assessments to identify client strengths, needs, risks, and service priorities.
• Develop and maintain individualized service plans in collaboration with clients.
• Provide short-term individual counseling, supportive counseling, crisis intervention, and other clinical services.
• Provide trauma-informed, culturally responsive, client-centered services.
• Identify and respond to high-risk situations involving mental health, substance use, safety concerns, and other needs.
• Monitor client progress and adjust service plans and interventions as needed.
• Advocate for clients and assist them in navigating healthcare, housing, benefits, and other systems.
• Maintain an active caseload of ~8–12 clients and coordinate move-ins, orientations, and wellness check-ins.
• Coordinate with Housing Specialist and staff to address tenancy concerns and promote housing stability.
• Provide crisis intervention, conflict resolution, mediation, and problem-solving support when needed.

Under the direction of the Sr. Director of Housing, the Clinical Client Care Coordinator (LMSW/LMHC) is accountable for providing intensive case management and crisis intervention services for clients. The Clinical Client Care Coordinator will work with individuals and families to promote recovery, resilience, wellness, and long-term housing stability through trauma-informed care, clinical assessment, counseling, crisis intervention, case coordination, and individualized service planning. The position will work closely with Iris House staff, housing personnel, government agencies, healthcare providers, behavioral health providers, and community-based organizations to ensure coordinated and client-centered service deliveryESSENTIAL POSITION FUNCTIONSClinical Services and AssessmentConduct comprehensive psychosocial and resident assessments to identify client strengths, needs, risks, and service priorities.Develop and maintain individualized service plans in collaboration with clients.Provide short-term individual counseling, supportive counseling, crisis intervention, and other appropriate clinical services.Provide trauma-informed, culturally responsive, and client-centered services to clients.Identify and respond appropriately to high-risk situations involving mental health concerns, substance use, safety concerns, and other behavioral health needs.Monitor client progress and adjust service plans and interventions based on changing needs.Advocate for clients and assist them in navigating healthcare, behavioral health, housing, public benefits, and other service systems.Supportive Housing and Resident ServicesMaintain an active caseload of approximately 8–12 clients.Coordinate client move-ins, tenant orientations, wellness check-ins, and other supportive housing activities.Conduct regular resident assessments and monitor ongoing progress toward individualized goals.Work closely with the Housing Specialist and other housing staff to address tenancy-related concerns and promote housing stability.Provide crisis intervention, conflict resolution, mediation, and problem-solving support when needed.POSITION REQUIREMENTSEducation: Master’s degree in Social Work, Mental Health Counseling, Counseling, or a related clinical field from an accredited institution.LMSW or LMHC licensure is required within 90 days of employment, in accordance with applicable requirementsExperience: Experience providing clinical services, counseling, case coordination, or supportive services to individuals and/or families.Experience conducting psychosocial or standardized assessments.Experience facilitating groups and developing or implementing group curriculum.
