Community Housing Innovations (CHI) is seeking a Case Management Supervisor to lead a team of case managers, ensure up-to-date service plans, and coordinate partnerships with community providers. This role focuses on stabilization, independence, and skill development for residents and includes supervising staff and ensuring compliance with program requirements.

Qualifications

• Education: Bachelor’s degree in social work or psychology required.
• 5 years of experience plus 2 years supervisory experience in social services for clients.
• Experience with immigrants and immigration issues prioritized.
• Seven years of case management experience and three years supervisory experience may substitute educational requirement.

Responsibilities

• Lead and supervise a team of Case Managers.
• Coordinate resources and partnerships to support service plans.
• Ensure plans are updated and aligned with resident needs.
• Advocate with external providers to secure services.
• Review discharge plans and ensure ongoing benefits enrollment.
