Telecare Corporation seeks a frontline case manager to help screen, assess, and link clients and their families to needed services during crisis. Under supervision, you will coordinate care and connect individuals to supportive resources within the program framework.

The role includes shifts on Fri–Sat with 7:00 AM–3:30 PM or 3:00 PM–11:30 PM, starting wage $23.00–$27.63/hour, with geographic pay differentials based on location.

Qualifikationen

• High School Graduation or GED with 3 years of direct services in mental health or community services; or an Associate's degree with 2 years of experience; or Bachelor's in Social Services.
• At least 1 year of direct service experience with Crisis Intervention, including screening and assessments.
• Sensitivity to multi-cultural populations and issues.

Aufgaben

• Provide screening, assessment, and linkages to services to members and families.
• Serve as primary support and case manager under supervision.
• Assist in crisis intervention and connect clients to appropriate resources.
