Qualifications

• Must be at least 21 years old.
• BA/BS or equivalent in human services or related field, with one year substitute for education.
• Experience in mental health, substance abuse treatment, or crisis intervention preferred.
• Pass a criminal history and background check; valid driver’s license and insurability.
• Competency in technology; pre-employment drug screen and TB screening as required.

Responsibilities

• Participate as an active member of the interdisciplinary treatment team.
• Meet with patients after admission to assess case management and discharge needs.
• Develop and implement individualized treatment plans with achievable goals.
• Initiate discharge planning early and update plans as needed.
• Coordinate follow-up services including behavioral health, medical, and housing.
• Promote continuity of medications and address barriers to access.
• Assist with accessing public benefits, insurance, and resources as needed.
• Collaborate with families, providers, and community partners to support continuity of care.
• Advocate for patients and provide clear discharge information and resources.
• Maintain timely case management documentation and protect patient confidentiality.
