Job summary

Care New England's Providence Center CSU team is seeking a dedicated Case Manager to conduct screenings, assessments, counseling, and referrals for clients. The role emphasizes crisis intervention and coordination with in-house and external services.

You will schedule appointments, assist with benefits applications, and complete required documentation while maintaining patient confidentiality and following organizational protocols. Some travel within the community may be required.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred in Human Services or related.
• Valid driver’s license, registration, current inspection, and insured auto.
• Strong knowledge of community resources and systems (housing, education, employment, healthcare).
• Bilingual preferred; language assessment within 30 days for bilingual roles.

Responsibilities

• Perform case management with in-house and external services.
• Maintain calm and professional conduct during crises or emergencies.
• Schedule appointments with outside agencies for clients.
• Assist clients with benefits acquisition (SSI, SNAP, PH).
• Complete required paperwork and progress notes in EMR.
• Transport clients to activities and appointments in Center or personal vehicles.
