Job summary

Rock Valley Community Programs, Inc. is seeking a Crisis Case Manager to deliver hands-on care and support to adults in short-term crisis stabilization in the Rock County area.

The role includes on-call rotation and daily individual/group sessions, with focus on trauma-informed, recovery-based practices. The position requires strong communication, judgment, and documentation skills, plus a valid driver’s license and background check clearance.

Qualifications

• Bachelor's degree preferred.
• CNA certification may be considered with relevant experience.
• Experience working with adults in crisis or with co-occurring disorders.
• Familiarity with trauma-informed care and recovery-based practices.
• Strong judgment, problem-solving, and communication skills.
• Working knowledge of computers, or willingness to learn.
• Valid driver's license and clean driving record.
• Must complete TB test, and drug screening (on-site).
• Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days.
• Employment is contingent upon passing a state and federal background check.

Responsibilities

• Provide individualized care that protects consumer rights, dignity, and wellness.
• Support individualized crisis stabilization response and service planning.
• Facilitate daily individual and group sessions (non-therapeutic).
• Document behaviors, interventions, and progress within 24 hours.
• Coordinate care and communicate updates with internal crisis team.
• Participate in staffings and professional development sessions.
• Maintain compassionate, professional relationships with clients and team.
• Maintain a minimum billing rate of 66% through timely and accurate documentation.
