Job summary

Highland Rivers Behavioral Health is seeking a Case Manager for the Crisis Stabilization Unit to deliver direct care, support discharge planning, and connect individuals to aftercare services. You will work within a multidisciplinary team to assess needs, coordinate services, and document clinical progress in accordance with agency policy.

The role requires a Bachelor’s degree in a behavioral health field and at least 2 years of psychiatric setting experience.

Qualifications

• Bachelor's degree required in Psychology, Social Work, Sociology, Human Services, Criminology
• 2 years of professional behavioral health experience, specifically in a psychiatric setting
• Criminal background check clearance, official transcripts, valid driver's license/ID, drug screen, TB test, and eligible 7-year MVR
• Bilingual proficiency in English and Spanish is highly preferred

Responsibilities

• Assist individuals with identifying aftercare needs and developing a discharge plan through interviews with the individual and family when appropriate, the treatment team and clinical staff
• Participate in the treatment team to assist with discharge planning needs
• Provide linkage to aftercare services by arranging appointments, transportation and supports
• Provide follow-up and assistance to improve engagement in aftercare services and reduce recidivism
• Monitor the individual’s physical and emotional wellbeing and report unusual behavior or physical complaints to the direct supervisor or Charge Nurse
• Assist in the admission and discharge process, including obtaining vital signs, safety search of individual and belongings, and orienting new individuals to the unit
• Maintain clinical records and documentation according to agency policies
• Assist in crisis prevention and management, including response to crises and use of seclusion/restraint as needed
• Provide direct care for individuals as needed
• Transport individuals to approved designated location at discharge
• Other duties as assigned
