Job summary

Pawnee Mental Health is seeking dedicated professionals to support individuals in crisis and help coordinate discharge planning and continuity of care. This role involves a mix of on-site CSU duties and off-site community/mobile work in a high-intensity environment.

The position emphasizes collaboration with a multidisciplinary team, strong English communication, and willingness to perform shift-based responsibilities including nights and weekends.

Qualifications

• Bachelor’s degree in human services or equivalent experience.
• One year of experience in mental health/crisis services.
• Strong oral and written communication in English.
• Basic computer skills for electronic records.

Responsibilities

• Collaborate with the interdisciplinary team to develop and implement person-centered treatment and crisis stabilization plans.
• Support discharge planning and safe community reintegration with follow-up care.
• Coordinate discharge and aftercare with external providers as appropriate.
• Perform CSU daily operations including rounding, documentation, and transport for clients.
• Link clients to ongoing behavioral health services and housing resources.
• Assist with crisis intake, stabilization efforts, and admissions logistics.
• Provide mobile crisis team support and field-based safety planning.
• Serve as shift Charge and triage needs across the department.
• Answer crisis line calls and conduct phone triage; document interactions in the health record.
