Qualifications

• Strong oral and written communication skills in English.
• Valid driver’s license and automobile liability insurance.
• Basic computer skills to enter and retrieve data in electronic systems.

Responsibilities

• Develop person-centered treatment and crisis stabilization plans with the interdisciplinary team.
• Support discharge planning and ensure safe community reintegration and follow-up care.
• Coordinate with external providers for discharge and aftercare supports.
