Job summary

You will provide direct, planned support in home, school, work, or community settings and collaborate with families and treatment teams to help clients reach their goals.

You will document progress in charts, participate in supervision, and respond to crisis as needed.

Qualifications

• Strong oral and written communication in English.

Responsibilities

• Provide direct services to consumers in home, school, work or community settings as prescribed by the person-centered plan.
• Collaborate with consumer, family and treatment team to support goals.
• Monitor and document progress in the electronic chart and ensure timely updates.
