Job summary

Paycom - ATS in Birmingham, AL seeks a Crisis Care Liaison to provide case management for individuals discharged from the Crisis Care Center. You will serve as the main contact during transitions and coordinate services with community providers.

The role requires a 4-year degree in a human services field, experience with mental illness or substance use issues, and a valid Alabama driver's license. Strong communication and reliable documentation are essential.

Qualifications

• Bachelor's degree in social work, psychology, counseling or other human service field is required.
• Experience with individuals with serious mental illness and/or substance use issues is preferred.
• Experience in case management or outreach is preferred.
• A valid Alabama driver's license and a suitable automobile are required.
• Must show and maintain car insurance per Alabama standards.
• Excellent oral and written communication skills are essential.
• Knowledge of community resources and services is necessary.
• Ability to obtain Adult Case Management Certification through the Alabama Department of Mental Health.

Responsibilities

• Provide direct case management services to individuals discharged from the Crisis Care Center.
• Be the single point of contact for persons transitioning from the Crisis Care Center.
• Identify resources and arrange transportation to appointments as needed.
• Prepare and document a plan for community mental health services for the client.
• Aid in obtaining and coordinating social and maintenance services including daily living, habilitation, rehab, prevocational, vocational, and housing services.
• Coordinate with community mental health organizations regarding client status and progress.
• Maintain high quality and timely documentation.
• Assist clinicians and case managers in discharge planning.
• Perform all required administrative duties.
