Responsibilities

• Coordinate discharge planning and warm handoffs with community providers.
• Assess needs and devise individualized service plans for each client.
• Develop and implement discharge plans and follow-up services.
• Serve as liaison between client and service providers for continuity of care.
• Document activities and maintain records in the electronic health record.
• Provide transportation to clients when necessary.
• Perform administrative duties as required.
