Job summary

Paycom - ATS in Alabama seeks an Intensive Care Coordinator to provide outpatient case management for seriously mentally ill clients, coordinating services with the treatment team to promote recovery.

You will assess residential stability, develop Individual Service Plans, assist with medication management, transportation, and access to resources, and document progress in electronic records.

Qualifications

• Bachelor’s Degree in social sciences field.
• Excellent verbal and written communication skills.
• State of Alabama Adult Case Management Training.
• Knowledge of Community Lay-out (Cullman County Area).
• Knowledge of Community Agencies and Resources.

Responsibilities

• Provide outpatient case management to the seriously mentally ill population.
• Provide direct services to severely mentally ill clients addressing unmet needs and continuum of care.
• Monitor and support medication adherence and awareness of side effects.
• Develop and implement Individual Service Plans and review progress quarterly.
• Coordinate transportation and referrals to agencies; maintain documentation in electronic records.
