Job summary

Horizon Health Services in East Amherst, NY is seeking a Clinical Care Specialist to support care coordination, crisis response, and medical records management. You will work with a multidisciplinary team and assist with educational activities for credentialed clinical services.

Qualifications

• Bachelor’s degree with 1 year Behavioral Health or related experience required.
• Associates degree with 2 years Behavioral Health or related experience required.
• High School Diploma with 6 years Behavioral Health or related experience required.

Responsibilities

• Provide care coordination by relaying medication statuses and following up on releases and forms.
• Work with multidisciplinary clinical teams to support patients.
• Conduct toxicology tests with patients.
• Respond to crisis situations at the clinic.
• Review records to ensure updates are completed timely.
• Support training and credentialing for clinical services.
• Interact positively with children/adolescents and families from diverse backgrounds.
