Job summary

Horizon seeks a Clinical Care Specialist in Buffalo, NY to coordinate care, support behavioral health services, and work with a multidisciplinary clinical team. The role includes crisis response, chart review, and follow-ups with patients and families in diverse settings.

This full-time position emphasizes training and patient-centered care within our community. The role is based at 77 Broadway St, Buffalo NY, with a pay range of $19.50 - $21.00 per hour, starting pay influenced by experience

Qualifications

• Bachelor’s degree with 1 year of Behavioral Health or related experience required.
• Associates degree with 2 years of Behavioral Health or related experience required.
• Associates degree with at least 1 year of Horizon Health site experience required.
• High School Diploma with 6 years of Behavioral Health or related experience required.

Responsibilities

• Coordinate care by relaying medication statuses and following up on releases, paperwork, and status update reports.
• Work with multidisciplinary clinical teams.
• Conduct toxicology tests with patients.
• Respond to crisis presentations at the clinic.
• Review and flag patient records for timely updates.
• Complete internal training and credentialing for approved clinical services.
• Interact with children/adolescents and families from diverse backgrounds in a positive, supportive manner.
• Prepare correspondence, reports, and forms using EHR, dashboards and reports.
• Provide support and triage for behavioral health and medical follow-ups.
• Attend Clarence Treatment Court weekly.
