Qualifications

• Education: High School Diploma or GED.
• Experience: Lived experiences of youth and community preferred, with ability to connect with youth.
• Licensure/Certification/Registration: Valid driver’s license.

Responsibilities

• Coordinate intake and engagement for youth referrals.
• Liaise between CHR, clients, families, and community providers.
• Link clients to behavioral health, medical care, housing, education, and employment resources.
• Assist with appointment scheduling and transportation coordination.
• Maintain accurate documentation of services and referrals.
• Participate in supervision and team meetings.
• Perform other duties as required.
