Job summary

Sun City Post Acute is seeking an Activity Assistant to support the Activity Director in planning and delivering a creative program that meets residents’ interests and needs while complying with federal, state, local, and corporate guidelines.

You will help schedule outings, arrange group and individual activities, document progress, and encourage participation in hobbies, crafts, and reading, all within a compassionate, resident‑centered care approach.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long-term care facility.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in providing communication between staff, residents, families, and others to meet resident needs.
• Participate in community planning related to facility interests and resident needs.
• Develop monthly activity calendar and maintain attendance records.
• Assist in developing and implementing action plans to correct quality deficiencies as requested by Activities Director.
• Assist with discharge planning and resident assessments.
• Arrange transportation for residents as needed for outings or appointments.
• Develop monthly activity schedules including outings, group activities, and in-room activities for bed-bound residents.
• Encourage participation in self-initiated activities including hobbies, crafts, and reading.
• Provide reading materials in Braille or audio books as appropriate.
• Assist with assessment documentation and quarterly progress notes.
• Keep Activity Department clean, orderly and secured.
