Job summary

Huntington Valley Healthcare Center seeks an Activity Assistant to support the Activity Director in creating a creative, interactive activities program aligned with regulatory guidelines. You will help plan events, coordinate communications, and assist with calendars and resident assessments to meet diverse resident needs.

Responsibilities include planning activities, arranging transportation, and aiding in discharge planning, while maintaining departmental organization and QA support.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in communication between employees at all levels, residents, families, and the public to meet residents' needs.
• Participate in community planning related to facility residents and services.
• Assist in developing monthly activity calendar and attendance records.
• Assist in action plans to correct quality deficiencies as requested by Activities Director.
• Assist with discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
• Develop monthly activity schedule including outings and in-room activities for bed-bound or isolated residents.
• Encourage participation in self-initiated activities and provide reading materials (Braille or audio books as appropriate).
• Assist with assessment documentation and quarterly progress notes.
• Keep the Activity Department clean, orderly and secured.
