Job summary

Centerville SNF Healthcare, LLC is seeking an Activity Assistant to support the Activity Director in running a creative and interactive activities program for residents, in accordance with federal, state, local and corporate standards.

You will participate in planning and conducting activities, assist with the monthly calendar, attendance, transportation, and documentation, while maintaining safety and quality of life for residents across the facility.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing good communication between employees, residents, families, support personnel, government agencies/personnel, and the public to ensure the needs and best interest of residents are met.
• Assist in development of monthly activity calendar and maintaining attendance records.
• Assist in discharge planning, development and implementation of activity care plans and resident assessments.
• Assist in arranging transportation for residents for outings or appointments.
• Keeps Activity Department clean, orderly and secured.
