Job summary

PACS is seeking an Activity Assistant to support the Activity Director in creating a creative activities program for residents. The role requires coordinating with staff, families, and agencies to meet psychosocial needs and ensure a compliant program.

The ideal candidate has a high school diploma (or equivalent) and preferably one year in long-term care, with strong reading, communication, and planning skills. The position is full-time and based on site.

Qualifications

  • Ability to read technical procedures and policy manuals.
  • Strong communication between staff, residents and families.
  • Reasoning to solve practical problems using written or scheduled instructions.

Responsibilities

  • Plan and conduct individual, small and group activities.
  • Communicate between employees, residents, families, and agencies to meet resident needs.
  • Contribute to community planning and activity calendars.
  • Develop monthly activity calendars and attendance records.
  • Assist in action plans to correct quality deficiencies as directed.
  • Assist with discharge planning and resident assessments.
  • Arrange transportation for residents as needed.
  • Develop monthly activity schedules including outings and in-room activities.
  • Encourage participation in hobbies, crafts, and reading.
  • Provide reading materials in Braille or audio books as appropriate.
  • Assist with assessment documentation and progress notes.
  • Keep the Activity Department clean, orderly and secure.
