Job summary

Family Guidance Centers, Inc. in Springfield, IL seeks a Counselor (Residential) to develop treatment plans, conduct intakes, and coordinate care for a full spectrum of referrals.

The role requires a CADC or ability to obtain certification, a high school diploma, and preferably a Bachelor's in psychology, social work, or counseling. Monday–Friday 8:00–4:30, 40 hours weekly, with pre-employment drug testing.

Qualifications

• IAODAPCA CADC certification required or obtainable within allotted time.
• State licensure (LP/CPC/LCSW) preferred, not required.

Responsibilities

• Develop patient treatment plans within 7 days of admissions and conduct weekly continued stay reviews.
• Provide patient intakes, treatment planning, case coordination, staffing, and discharge with individual and group therapy.
• Maintain at least 50% direct patient engagement time.
• Complete and maintain accurate records or reports on patients' histories and progress.
• Provide documentation for insurance submittals to obtain coverage.
• Assess drug dependency by collecting/analyzing drug-screening samples.
• Interview patients, review records, and confer with professionals to evaluate suitability for programs.
• Review progress and develop high-quality notes and insurance progress items weekly.

Essential Duties

• Develop patient treatment plans (within 7 days of admissions) and conducts continued stay review (weekly) based on research, clinical experience, and patients' histories - using SAMMS assessment on computer. Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
• Maintain at least 50% of work time proving direct service/face-to-face patient engagement.
• Complete and maintain accurate records or reports regarding patients' histories and progress, services provided, or other required information.
• Provide detailed documentation for submittal to insurance companies to obtain coverage.
• Assess individual's degree of drug dependency by collecting and analyzing drug-screening samples.
• Interview patients, review records and confer with other professionals to evaluate individual's mental health and physical condition and to determine their suitability for participation in a specific program.
• Review and evaluate patients' progress in relation to measurable goals described in treatment and care plans, developing high quality continued stay reviews and insurance progress notes weekly.

Schedule

Monday-Friday 8:00 AM - 4:30 pm. Schedule may vary, must have flexibility to meet case needs. Must work a minimum of 40 hours per week.
